Meadowview Christian Board of Directors is ever mindful of the dangers drugs and alcohol pose on the health, safety and well being of students and stall. In accordance with the Federal guidelines, a is necessary for an individual that has not reached his/her eighteenth birthday to have parental consent prior to the administration of a drug and/or alcohol test. In consideration for accepting my child as a student at Meadowview Christian School, Inc., I do hereby agree to the following:

I understand that this is only an application for admission to Meadowview Christian School. This application is subject to review and acceptance by the Meadowview Christian School Board of Trustees. Acceptance of this application for the school term and for the retention of the student for the said term shall be based on past and current academic performance and social behavior.

I understand that acceptance of my child(ren) as a student by the Meadowview Christian School Board of Trustees for any academic year does not qualify my child for acceptance as a student for any other academic year. I understand that this application and acceptance only applies for the school term June I, 2022 through May 31, 2023. My child may be dismissed during the school term June I, 2022 through May 31, 2023 for poor academic performance, improper social behavior, or any rule violation deemed by Meadowview Christian School Board of Trustees in its sole discretion to warrant expulsion. In this event it would be mv responsibility under this contract to pay in full all school tuition, fees, and related expenses. ** In the event Meadowview Christian School Board of Trustees does not accept this application, the Registration Fee will be refunded.

I understand that tuition is due on the 1st of every month and late after the 10th at which time I will be charged a late fee of $50 per student. I also understand that account information will only be discussed with person/persons who have signed this contract. I further understand that on the 45th day of non-payment my child will be suspended until ALL arrears are current; this includes monies owed to any part of MCS. I also understand that my child will not be able to participate in any extracurricular activity (sports, cheer, pageant, etc.) if my account is more than 30 days past due.
Report cards will be held if my account is past due more than 10 days.

I understand that my child will comply with all school regulations and that it is my responsibility to understand these regulations and insist on such compliance. I further understand that my child may be suspended or expelled from school if he/she refuses to abide by the regulations. In such case, tuition and fees are
to be paid in FULL.

I understand that each family with a child in grades K5-12th must contribute a minimum of (15) hours per semester of time working at the school. In lieu of this time, the family will pay $100 per semester ($200 annually). I understand it is my responsibility to contact the school for work dates/times/etc. Work cards are due November 2011, and April 20th. It is the parents' responsibility to turn in work hours. If not turned in on time fee will be assessed.

Fundraising: Each family is required to sell $200 worth of tickets 2 times a year. In addition, each family is required to sell or donate $100 towards MCS Golden Ticket Raffle.

In registering my child for the 2022-2023 school years, I do hereby pledge and promise the payment of all school tuition, fees, and related expenses. I understand that the registration period held during the spring is for the school year beginning in August of that same year. I understand that it is the policy of the school to make no refunds of registration fees for
students who are accepted at Meadowview Christian School. After a student has been accepted, an early withdrawal fee of $500 per student will be charged prior to August 1st, After August 1st, all tuition and fees for the entire year are due to the school regardless of the child's attendance.
